Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, automobile, disability or workman’s compensation type insurance are involved, this discount plan cannot be used.
This plan is nonrefundable. No refunds are given if patient chooses not to use their plan. Plan is subject to change yearly. If you choose to use a medical payment credit card to pay for dental treatment while using this plan, the discount will be reduced to 15% due to merchant transaction fees
Dental Discount Plan is not insurance: it is a reduced fee plan for services provided in this office. It is a reasonable, cost effective alternative to dental insurance.
Everyone is eligible to participate in the plan.
Your plan membership will begin immediately. Since we administer this program in office, there is no need to wait for notification of eligibility or an insurance card. You simply make your appointment and pay the discounted plan fee at the time services are rendered.
The Dental Discount Plan is not dental insurance and is not designed to coordinate with any other dental coverage. It cannot be combined with any other dental insurance or special pricing.
The Dental Discount Plan can be used for procedures provided in this office and our participating endodontist and oral surgery offices.
